Accident Summary:

 ON 30 MAR 70, ARMY AIRCRAFT SN: 65-9512, A UH-1C, ASSIGNED TO THE 155TH AVN CO (AH), DEPARTED BAN ME THOUT, RVN WITH ITS TRAIL SHIP TO COVER AN EXTRACTION OF TROOPS AND RICE INTO BUON HO, RVN, WHICH IS LOCATED APPROXIMATELY 20 MILES NNE OF BAN ME THOUT, RVN. UPON ARRIVAL AT THE BUON HO AIRSTRIP THE TWO AIRCRAFT ESCORTED THREE LIFT AIRCRAFT TO THE LZ AND THEN RETURNED DIRECTLY TO BAN ME THOUT TO REFUEL. AFTER REFUELING THE TWO AIRCRAFT AGIN RETURNED TO BUON HO. AT THIS TIME THEY WERE CONTACTED BY FAC, THAT WAS ON STATION IN THE AREA, AND ASKED TO EXPEND ON A SUSPECTED TUNNEL COMPLEX. BECAUSE OF A TEMPORARY HALT IN THE EXTRACTION, THE GUNSHIPS PROCEEDED TO MAKE THREE RUNS AT THE TARGET. AIRCRAFT SN: 65-9512 EXPENDED ALL BUT THREE ROCKETS BUT HAD ITS ENTIRE MINI-GUN LOAD LEFT. AT THIS TIME THE TWO AIRCRAFT RETURNED TO BUON HO AND SHUT DOWN. THE TRAIL GUNSHIP DISCOVERED HE HAD A TRANSMISSION OIL LEAK AND PERMISSION WAS OBTAINED FROM THE GROUND COMMANDER FOR THE TWO SHIPS TO RETURN TO BAN NE THOUT. WO1 JONES, THE CO-PILOT, PROCEEDED TO MADE A NORMAL TAKE-OFF FROM THE GROUND, USING 96% N1 AND 6400 RPM. AFTER INITIAL TAKE-OFF THE PILOT INSTRUCTED WO1 JONES TO VEER SLIGHTLY TO THE LEFT TO AVOID OVERFLYING AIRCRAFT THAT WERE PARKED ON THE EASTERN END OF THE AIRSTRIP. AT THIS TIME THE THREE (3) REMAINING ROCKETS IN THE LEFT POD SALVOED, ONE OF WHICH LANDED IN THE VILLAGE OF BUON HO, WHICH IS LOCATED AT THE EASTERN END OF THE AIRSTRIP. WO1 JONES IMMEDIATELY APPLIED AFT CYCLIC TO PREVENT ANY FURTHER ROCKETS FROM LANDING IN THE VILLAGE, NOT REALIZING THAT HIS TUBES WERE EMPTY. CW2 DICKINSON GRABBED THE CONTROLS JUST AS THE AIRCRAFT ROTATED 90 DEGREES TO THE RIGHT AND THE LOW RPM AUDIO CAME ON. AS THE AIRCRAFT STARTED DESCENDING IT CONTACTED TWO TREES WITH NO APPARENT MAJOR DAMAGE TO THE AIRCRAFT. REALIZING THAT HE WAS HEADED FOR SOME HUTCHES. CW2 DICKINSON DIRECTED THE AIRCRAFT TOWARD THE RIGHT WHERE HE HAD SPOTTED AN OPEN AREA. AT APPROXIMATELY 50 FEET AGL HE EXECUTED A FLARE AND JUST PRIOR TO TOUCHDOWN THE AIRCRAFT TURNED 180 DEGREES TO THE RIGHT. THE AIRCRAFT IMPACTED TO THE GROUND ON THE FRONT PORTION OF THE LEFT SKID. THE CREW CHIEF UNBUCKLED HIS SEAT BELT AND ATTEMPTED TO JUMP CLEAR OF THE WRECKAGE. HOWEVER, THE AIRCRAFT CONTINUED TO ROLL TO THE LEFT AND CAME TO REST ON ITS LEFT SIDE PINNING THE CREW CHIEF TO THE GROUND. THE OTHER THREE CREW MEMBERS EXITED THE AIRCRAFT AT THIS TIME AND WITH THE HELP OF PEOPLE WITNESSING THE CRASH, FREED THE CREW CHIEF FROM THE WRECKAGE. THE MAJORITY OF THE WRECKAGE WAS FOUND WITHIN 10 METERS OF THE AIRCRAFT ITSELF. HOWEVER, PIECES OF THE LEFT CHIN BUBBLE WERE FOUND APPROXIMATELY 350 FEET AWAY FROM THE WRECKAGE AT THE BASE OF THE TREES THAT THE AIRCRAFT HIT. ALSO, APPROXIMATELY 14 FEET OF ONE OF THE MAIN ROTOR BLADES CUT THROUGH THE ROOF OF A HOUSE THAT WAS LOCATED APPROXIMATELY 250 FEET FROM THE WRECKAGE.\\
