Accident Summary:

 AT APPROXIMATELY 2210 HRS, UH-1D SN 64-13705 ALONG WITH THE OTHER SHIPS LOCATED AT DUC HOA DEPARTED AND RENDEVOUZED WITH THE SHIPS FROM HARDCORE FSB. COMMAND AND CONTROL SHIP PASSED ON MISSION INFORMATION TO THE FLIGHT LEAD AT THIS TIME. THE LZ, A RICE PADDY, WAS TO BE LIGHTED BY THE LIGHT SHIP, AND SUPPRESSIVE FIRE TO BE PROVEDED BY THE GUN TEAM, UTILIZING ROCKETS. THE FLIGHT ENTERED ON FINAL APPROACH WITH THE GUN TEAM ESCORTING THEM. ON SHORT FINAL THE LEAD SHIP TURNED HIS LANDING LIGHT ON, WHILE THE GUNSHIPS SUPPRRSSED THE LZ WITH ROCKETS AND MINIGUNS. THE LIGHTSHIP WAS OVERHEAD, CIRCLING AND LIGHTING THE LZ. AT THIS TIME CHALK 2 AIRCRAFT (705), WHO WAS BEHIND AND BELOW THE OTHER TWO SHIPS IN THE FLIGHT, FLEW INTO THE GROUND COMING TO REST APPROXIMATELY 200 METERS SHORT OF THE INTENDED LZ. THE AIRCRAFT HIT INITIALLY IN A STEEP FLARE WITH THE TAIL ROTOR STRIKING THE GROUND AND SEPARATING FROM THE AIRCRAFT. THE HELICOPTER THEN BOUNCED INTO A NOSE LOW ATTITUE HITTING THE GROUND WITH THE MAIN ROTOR BLADE AND FINALLY COMING TO REST ON ITS RIGHT SIDE. THE UH-1D FLYING THE THE CHALK 3 POSITION NOTIFIED THE LEAD SHIP, WHO IN TURN NOTIFIED THE C&C SHIP OF THE ADCIDENT. THE LAD SHIP TERMINATED HIS APPROACH SHORT OF THE PROPOSED LZ, IN ORDER TO SECURE THE DONNED AIRCRAFT. THE LIGHTSHIP SHIFTED ITS LIGHT TO THE CRASH SCENE TO AID IN RESCUE. CHALK 3 AIRCRAFT HOVERED NEAR THE DOWNED HELICOPTER AND AIDED IN RESCUE OF THE CREW AND PASSENGERS, THEN FLEW THE INJURED PERSONNEL TO THE 93RD EVACUATION HOSIPTAL IN LONG BINH.\\
